A Massachusetts assisted-living center where nine residents died in a fire was cited for failing to immediately report more than two dozen health and safety incidents.

Flames tore through an assisted-living facility in Massachusetts, killing nine people and injuring at least 30.

River firefighters arrived at the scene Sunday to find some of the trapped residents screaming for help out of the windows.
